Our Services

The Specialist Palliative Care team (The Team) based at Ty Olwen provides a service to Singleton, Morriston, Neath Port Talbot and outlying community hospitals. It also provides a community service to the population of Swansea and NeathPortTalbot.

The service provides a range of specialist support. Our_Services_clip_image002These include:

  • Advice with managing symptoms
  • Psychological support for patients and families
  • Information and education support for patients and families

The care is delivered in partnership with the general practitioners, district nurses, hospital based doctors and nurses. The Team consists of doctors, nurses, physiotherapists, occupational therapists; designated social worker, chaplaincy service, administration and volunteer staff.

The following Specialist care is offered in the following settings depending on the needs of the patient.

Ty Olwen Inpatient Unit

The inpatient unit provides specialist palliative care for patients who have changing complex care needs.
Following admission a plan of care is made with the patient. This includes:

            Future care planning
Discharge
Ongoing symptom control
Transfer to residential and nursing homes
Transfer to other hospital settings

Hospital Team

The hospital team will visit general wards following a request from doctors whose ward and care a patient is under. The team will provide specialist advice and support in the hospital setting working closely with the ward doctors and nurses.

Community Team

The community team provides specialist advice and support in the patient’s own home or at out patient clinics working in partnership with the general practitioner and district nurse. The community team will also provide specialist advice and support in residential,  care and nursing homes.

Day Care Service Team

The day care team provides specialist advice and support at the day care unit in     Ty Olwen, working in partnership with the general practitioner and the district nurse.
